Our clinicians are happy to discuss requests for sick notes / fit notes. These can be requested using our E-consult service. You will find a link for this on the front page of this website and below. If you cannot access this please call our reception team who will take your request.
When submitting a request for a sick note, you may be asked to provide the dates of illness and a brief explanation. Please ensure you have this information when you request your certificate, this will be required whether it is your first or follow up certificate.
