New Patient Registration
You can come to the practice to fill in a registration form, new patient questionnaire and consent forms for record sharing and online access.
We always prefer to see 1 form of ID for all newly registered adult patients. This helps with any spelling and date of birth queries.
Ideally we would see photo ID and proof of address in the form of a utility bill, council tax letter, bank statement within 3 months of issue will be accepted. If no photo ID is available a birth certificate/marriage certificate or similar will be acceptable.

For online Access to your medical record: we need 2 forms of ID, one must be photo ID please.

Proxy access for parents, from April 2020, is granted (with evidence of parental responsibility) until the child reaches 11 years old. Access will then automatically cease. Between the ages of 11- 16 years old, this access will NOT be reinstated.
